I am wondering if you can please take a look at this code and let me know what I need to change....
I want the information the user enters on the form to be emailed to me. Also, I want to be able to open that form on my blackberry as well.
When I first created this form I got everything to work fine but then changed something which takes me to my current position.

See code below. Thanks!!!

At the top of your code you have:

<form action="/gdform.php" method="post">

I suggest taking those four lines out, as they could be the problem there.
