Say I have an image on my server and someone is using it by using the code:
<img src="mysite.com/pic.jpg" />

I know that you can track the number of times this picture has been loaded, but can you track which page it was loaded on? Can you find out any information about a user that is visiting the page such as ip address?

It depends on your hosting provider. For example, Godaddy has access logs that you can view and see the IP of every hit to your page. You could just scan for an IP that accesses the image without accessing any page on your site.
