Just dumping a bunch of error messages on a help forum isn't useful. And posting them without the relevant code is pointless.

The only thing we can tell you about them is you have array indexes and variables that don't exist at the time your code runs. Either they don't exist yet, because nothing has been submitted and your code isn't first checking for a form submission or they don't exist at all because your form doesn't contain those named fields.

Even the message you posted in the title isn't the complete message and if you did read or post the complete message, along with the code, it would help to find what's causing the error. However, since I know what the complete message likely is, it means that you haven't met the relaying restrictions for the mail server, that at least one of the To: or From: email addresses are hosted at the sending mail server or your mail server unconditionally requires using smtp authentication. Given the other errors, it's likely that a completely missing/empty email address is the cause of this error and it is only a follow-on error caused by a problem somewhere else.

The last error message, without knowing the exact code and the context where it was produced, is also pointless. Best guess is there was no To: email addresses supplied to a function call.

Lastly, this has nothing to do with Apache Configuration. Apache is a web server. You would post in this forum section if you had web server configuration problems. What you have is primarily a php code problem and possibly a need to interface with your mail server using the method that it requires.

Are you the same user that created this account on this forum? You have 400 posts in a 3.5 year span. That's an average of about one post every 3 days, for 3.5 years. You should be able to write php code and solve simple undefined index/variable errors in it, post the relevant information, and identify that you have a php coding problem.
Finding out HOW to do something is called research, i.e. keep searching until you find the answer. After you attempt to do something and cannot solve a problem with it yourself, would be when you ask others for help.

    php.ini hardly ever needs changes.
sendmail or postfix configuration depends on the server. If you are the owner of the server then you should know the correct parameters to setup the mail forwarding.
If you are not the admin one would assume mail() is configured for you. Never send direct smtp messages with PHP. They have a high risk of disappearing in case of error.
