A form that user can have their say & update it on line !

Dear All

I would be so grateful if you could help me in the below subject:

I needed some kind of form to put on a web site's page for the users to send their feedback and put it on that page, just like how we are all using this forum and putting any thing by typing and submit it,but I am talking about a simple kind of form that users can not edit or delete what they say. I do not know if there is anything like that, but I thought I should ask your help guys and I will certainly appreciate anything from you in the above regard.

How familiar are you with server side scripting? Processing submitted form data, storing it in some sort of way (could be as simple as a flat text file) and generating a result page to show the submissions sounds like a server side script to me.
