Attach file to online form...

Hi,

Have an online form, user fills in fields etc. etc. but there is another field which is for attaching a word doc etc. How do I do this so that when the user then hits submit it send the doc with it like and attachment?

i don't know if they mention it in the tutorial but you need to set the chmod on the folder the files are being uploaded to.
