On line Form

I set up my first website a couple of years ago and at that time asked quite a few questions here and always got great answers.

Now, having done no websites in the meantime have another idea which I want to try.

I need an online form, to capture
Name
Address line 1, 2, 3 and maybe 4.

email address
message

Previously I managed to find a free online html code / php script which worked great, but that was only for name, email and message.

Now I need more information and I am again looking for help, if this is possible.

I have read a bit about php, but there is no way in hell that I would be able to write my own program.

Has anybody got any ideas please.

Of course, if I really had to, I could use my existing form and simply ask readers to type everything in the message box ... but that would probably look a little amateurish.

I assume to an email, but where else can it go to ?

I want to take information from visitors to a website, then upload only a small amount of it to another part of my site. I will keep the rest of their data, to use later.

Is there a better alternative to an email for the data. I don't know anything about databases etc, so anything involving MySQL or whatever it is called, is out of the question.
