1) Project Details: (be as specific as possible):
Need an expert at php/mysql/javascript to troubleshoot/rewrite a complicated html form, then write php code that loops through each row of the form and saves the data to a mysql database. I have the database already, and have some of the form (but it needs more work), but don't have the script that will process the form results. If you can do this, will send explanation of what I am trying to accomplish, along with screen shot of form.

2) Payment method/ details (Paypal, check? Timeline?): Need this today. Will pay $200 via paypal.
