Simple PHP Order Form

1) Project Details: (be as specific as possible):
I need a simple PHP powered order form put together. Please see attached Excel mock up. All the expected validation should be in place. I will also need a security question option in place to protect against spam bots. Upon submission the order form data will be sent to an email address. It is crucial that the email can be easily read by the client.

Note: No styling is required as this will be done by myself using CSS.

2) Payment method/ details (Paypal, check? Timeline?): The completed order form will be required in two weeks' time. Payment can be made either via online banking if coder is in the UK or via Paypal.

The successful applicant will be paid in full after demonstrating a fully working version of the form.
