Contact Form (Flash) need help please

Hi, I have not touch flash in years and decided to brush up on it once more. I notice I have forgotten a lot of things. I created the look that I want in flash and very proud of myself. It took me a couple of months because it took me a long time to figure it out for the look I was aiming for. But got there thank goodness!

The problem I am having from a couple of weeks now is the contact form. I am having problem what to do next once a person hit the submit button. I don't know how to write the action scripts nor the php script in dreamweaver. I am at a total lost.

this is what I have for my instant names:
names
email
mobile
man (radio button)
woman (radio button)
agegroup (list)
fotoshooting (checkbox)
modelfotoshoot (checkbox)
comment
submit (button)

Could someone please help me on how to write an action script for these instants to function and for the information to come back to me via email? Also one for dreamweaver only if one is needed. Thanking you in advance. I have been working on this form for over a week and still can't figure it out

This is pretty easy to accomplish but you must have a minimum level of programming skills which I am afraid you don't.

Thank you djpaul for the link. I read it yesterday and been trying. The radio button, checkbox, and combobox is giving me error because I don't know how to code it and I am sure they should have if else clause. I plan to play around with it again today to see if I can get it too work. But the link you shared is very nice and I love how it break down the reason why. If I don't have it working by tomorrow. I come back online Sunday and let you know whats going on. Once again thank you so much.
