Exclamation Mimicking a particular web form

I have a test site up at www.trendstudiosorlando.com/test3/index.html

I was told to put a form on it that mimicks the look of this form on www.eoinn.com,
So far this is what i have, but im not entirely sure how to size it correctly and position is perfectly as such.. And as you see, currently, its shifting quite a few items to the left..

Any tips?

Why not just copy over the HTML and CSS from the source to get the general structure and size, then once in place style it to meet your websites design?

By the way, I just had a quick look at your site and the main menu is out of place. You might want to look into that (I'm using FF). Would have a look but just about to go out.

I tried that, but I cant figure out whats what as it *seems* like a million different plugins and besides the layout, i cant use any of it
