Datepicker problem

I am creating datapicker with the help of following link

datepicker on above link works fine but when I apply code to my page then it does not work.

The attachment is my html and other files.
Please help me to locate error.

Can you provide a link to your page? I'm not keen on opening unknown zip files and others will no doubt feel the same.

My page is not uploaded. It is in zip. I beleive that nothing is doubtful material included.
