webform plus logging

I need to create a online form for a user to enter some details eg

Person enters in name, email address, address and postcode

Email addresses are populated from a field based on postcode

Adds subject based on based on head user of that postcode eg "to bob"

Email is Sent to bob + emails based on postcode

Willing to pay anything at the moment up to 200$

Can do paypal, CC, bank deposit

an extra item that needs to be added is that it all needs to be logged so we can see how many people sent a email and their details.

So the full recap is here:

1) user go to www.mysite.com.
2) enters name,email,address,postcode.
3) the 'database' will search the csv's i have made to the postcode select the required member and sub emails it needs to send to.
4) change the message to be sent to include the name of the user that is boss of that postcode.
5) email is sent to postcode boss and all his sub users.
6) the email,name.address,postcode is logged so i can see what has been sent.
