Need simple flyout for site

1) Project Details: (be as specific as possible):
Need A bar that catches the persons eye and slides out when they click it so they can sign up for the newsletter which I have already I just need you to put it on some kind of slide out thing. I need this to be professionally done. Please experts only. I need this to match my site like it was all done together.

2) Payment Amount: $50 (I'm 18 in college with no job so sorry if I can't pay much. If you won't do it without more pay just talk to me I may be able to offer a bit more but not much)
