need email submit forum for website

i am asuming this is the correct forum....anyway.
i need a form for someone to send an email to me from my website.
i need it to have the following things for people to enter:

Code:
Screen Name/Nick Name
Age
Sections
Section information
Pictures
Games
Guestbook
other
each with its own entry field, then the submit button, when cliked, will email everything to a specified email address
*****and, if possible, the "pictures" field should have an upload option*****
one that is EASY! to edit for someone who knows not to much about php/whatever the form uses...........

so show us the code? cause this looks like a bunch of text fields you would want with the exception of javascript in the "picutres" and the "submit button" parts. Otherwise this is pretty basic. The hardest parts would be those javascript parts...

I suggest you start learning php php isn't a language where you can just copy and paste scripts. Anyway, here's an email script tutorial, learn that and then modify it to your needs. If you need help modifying it, show us what you've done.
