Exclamation page details gone after clicking back

Hi. I created a form using javascript and html. On this page, it allows the user to enter details. There is a section where it allows the user to click and a javascript calender will pop up to select desired date. At any time when the user uses this calendar, after filling in the form, he then goes to the next page. But should he decide to return to the previous page, all records will dissappear. But if the user did not use the pop up calendar, it will be fine.

I also have another similar scenario. I have a form which consists of 2 pages. Page one is simple form entry. In page two, there is a section which asks the user to make a selection in a dropdown box (the record is retrieved via php), after selecting it and if he decides to go to the previous page, the past records exist. However when he clicks the forward page, the records in the drop down box dissapears. He has to input them all over again.

can someone help me?
