Form Php Script

I was told to post a topic here since my other post in the html section didn't help me.

Thread from the html section.

I have uploaded the dynaform.php & example.html files in .txt form. Please review it to see if I didn't everything right. Because as of right now, when I sumbit the form, I receieve any e-mail.

Is this due to my hosting? Please help. If so can you provide me with a free-hosting that would allow this to work. Thanks again.
