Layout Help (Using gridbaglayout most likely)

I'm designing a little java application that i want to look something like this:


i've tried using the GridBagLayout, but i can't seem to get it to work. If someone could create some basic code that would get me something close to that i would greatly appreciate it... i can take care of the little tweaking and minor adjustments. Thanks.
