Well, here's the thing. On a basic level, the php page will display the information on that page. So you can't really copy the url and expect to come back to the page with the information still present.
IOWs, if you have form.html POSTing the information to process.php, then your resulting url will be:
Simply going back to that page will either result in an invalid page, or a reprocessing of whatever POST information is contained. (Not sure about exactly what happens here server side).
What you would need is for the php script to generate and html page, then save it with a unique name so that it can be accessed continually.
Unfortunately, I'm not experienced enough to send you in the right direction. I could help you process.php displaying the information, but that's about it.
I can suggest one thing though.

Email address: <input type="text" name="email"><br><br>
(I bolded the parts I'm referring to)
I would change the name values to values without any spaces in them. I'm not positive, but pretty sure it will result in your php script not working properly if you try to implement spaces. In the best case, it will get messy, really quickly.
