Help with SimpleCart.js email checkout?

Hi There,

I have been reading lots of threads and code suggestions to make Simplecart checkout by sending an order to an email but I must be doing something wrong because I'm not sure why it is not working.

I have installed a view cart button at the top of the page and a shelf item on this page as my starting point following the guidance from the website:

which then points to my checkout page:

I started putting in some other form fields here that I found from someone else's code to populate the customer details, and a form action button to submit to sends.php, but nothing happens. I added Javascript code at the bottom of simple cert.'s - me.emailCheckout = function enquire() { , that is supposed to action the email method and added some script to the checkout page again requesting email checkout. I'm stuck! Can anyone please help? I'm not sure if it is something that I have set incorrectly in the javascript files or whether the PHP file I have created is wrong. I have spent hours and hours trying to figure it out, and would appreciate if anyone can help me?
