Rating and commenting form PHP

Hi i am making a website where the users can rate a product on 5 different qualities and comment on 4 other qualities, these ratings are then published on another site like a comment section, but i dont know how to code it. I know some php and know that i must use a database to store the input so they can later be displayed as comments, but that is all i know.
