Does anyone know of a tutorial (or a place where I can learn) to set up a feature on my website where visitors can "Recommend This Site" via an e-mail form, to a friend?

I have a good working formmail.pl script, and I was wondering how I could go about using it in an e-mail form where the visitor puts in his name & address, and then puts in his friends name & address, then clicks send and my pre-set message is sent along with my URL.

Any leads on how to do this would be great. Thanx!
