Your best bet would involve a something server side (PHP, CGI, etc.) as client side JavaScript would give you very limited options. What does your host support terms of server side?

Aside from that, JavaScript Kit offers this: http://javascriptkit.com/script/cut132.shtml which has apparently been changed and now uses a service called freepolls.com. Again, your best bet would be something server side, but if that is not an option available to you, freepolls might be worth considering. Good luck
