    Question How to collect employee request information...

    I am to develope a portal for my office administration, the purpose of which is:

    1. If an employee wants to request soemthing (leave, hotel reservation, car request etc), there are pre-designed forms (paper), which are currently being used as hard copy, now my boss wants them to be electronic form, so that If someone wants to submit them, he can do it online...

    The problem is: If I make all these forms as html form, and stored in db, I will have to make each form seperate, and there will be lots of fields.... is there any other way to submit a request using pre-defined forms we have.... or will I have to prepare each form as html, and store values to db...

    I shal appreciate any suggestion / solution...

    I would imagine that many of your forms are going to have the same fields, such as Name, Phone Number, and Address. I'd suggest creating your PHP to reuse those same fields and filter out what isn't needed based on which form they are submitting. That should save you quite a bit of time.

    I am creating empinfo table seperate, and departments tables as well, and then I want to create applications table, with appID, appTitle, etc.. but i am afraid that there will be lots of fields, mixed up for each form.... I was wondering if I can use pdf forms, and after a form is filled in and submitted, a new pdf is generated with filled in fields, and save in the user's profile, and also a copy to manager or reporting person.... but I don't know if it will work or not.... any ideas...
