Send fail failure
We've made an html form and are trying to use php to get it working but it appears to not want to work at all, also just to make you aware we are real laymen at php too it's a wonder we got as far as we did! ;-)
Can someone please help with the attached code.
We can't seem to get it to work at all, we think we've programmed it correctly but not being experts we are not 100% sure if it's something we've done or it's something else.
The html form and the php is attached in the zip file.

You have to describe what you mean by "doesn't work at all".
Would that be a PHP script error?
Or it displays something, but no email is sent?
What do you mean?
Maybe your webhost doesn't allow PHP mail.
Maybe your webhost's version of PHP doesn't allow $_REQUEST.
$_REQUEST is deprecated ... they should all be $_POST
