perlmodstyle Perl modules: how to write modules with style
perlmodinstall Perl modules: how to install from CPAN
perlnewmod Perl modules: preparing a new module for distribution
perlpragma Perl modules: writing a user pragma

If I also may, you will also want to visit Perlmonks. They have an excellent section on their tutrorials page that is pertaining to Perl module creation. The documentation they have written is quite good and definitely worth referencing and reading.
