Arrow Javascript form calculator in existing webpage

1)I have an existing page that I need to add another calculator to. I have access to Javascript formula and code but it has to be deciphered down to the components I need only. The form part has to be made also, but is pretty simple. The calculator involves putting in two values, a calculate button, and output of 2 values.

2)PM me with your bid, but I am thinking the $100 to $150 Australian mark

    3) Payment method is Paypal

4)Pretty simple to do as existing page format can be followed, however knowing how to write the javascript and form, from the code out of the reference page is the key.

I'm interested. I'll send you a PM with my contact info. Also what is your timeline? When do you need this by? Do you have the existing page online somewhere? What formula do you have?
