    <form enctype="multipart/form-data" name="myform" .....

    <input type="text" name="email" .....

    <input type="text" name="hometown".....

    You need a specific name for each of your elements.

    (If the returned e-mail is mixed up and on one line, use

    enctype="text/plain" instead.)
