    Heres my Form

    Ive spent today designing and building it with Frontpage & Dreamweaver.

    Ive used Response-o-matic.com to send all the Form entries to me in an Email

    Whats everyone think of my Form?

    Have made it so people can complete it to use my Service.

    I would have used <label> instead of <th scope="row"> for the labels and you could add a <fieldset> and <legend> to improve accessibility, too. Otherwise I'd say it's nice—nothing special but it works.
