    HTML code error - forms

    I am trying to set up my site so that users can complete a brief form and then submit it to me. I went to my provider and setup the form and copied the raw html code onto my site. I tried it and it does send me an email but the message I get says - "Enter template. Use to mark where to put data."

    Here is the code if it helps. Obviously something is not right - any help would be appreciated.

    Form handler

    Appearently, the form handler--forms.cgi--requires you to supply a template so it knows what to do with the posted variables.
    If you have documentation for said form handler you ought to be able to find out how to use it.
