    creating a box and radio button to perform an action

    i was hoping someone can assist on this one
    I am tryin to do this


    i want a box with a radio button

    what i am trying to do is this

    someone can place an ip address in a box and when they hit submit then on submit it will perform the action of adding a port onto it and sending it to like a page

    so it would look like this

    Please type in ip: *BOX* *Submit Box*

    then it would take that person here http://ip address:100

    this can be done with any ip address that is placed into the box

    can anyone help here???
