    Survey

    I would like to make a survey for those who visit my website to fill out.

    I want to have around 10 questions, with 4 radio button answers they can pick from. Each answer is worth a number of points.

    At the end I would like all their points to be added up for them.

    How can I do something like this and put it onto an html page?

    Create an HTML page with a form. If you are unsure how to do this then Google "html tutorial".

    Use PHP or some other server-side language to process the form. If you do not know PHP, then learn.

    You can also use Javascript but it's less secure and not everyone has Javascript turned on.

    i know how to make a form where the results are sent to me, but i want the results to be sent or shown to them

    Do a google search for online poll applications. There are a bunch which are free and will either host the code/poll for you, or give you code to place on your site.

    if you want to show the person what they just fill out you can just use php echo statements, id suggest a php tutorial on forms. try http://www.tizag.com
