    Calling an external html form

    I am trying to write an external form and call it up in html or javascript, the one that would be the simplist. i wrote the form in dreamweaver and it has about 30 drop down boxes and 20 text boxes all with one submit button. i want to place this form on a page already on my site (form will change alot that is why i want external).
    my external file starts off with the form action/method tag and then goes straight the the select items. should i have document.write for ever line to use java script or just use html.
    i saved it two ways
    groupform.js
    groupform.htm

    and am trying to call it by placing a html fragment on the page i want to place it on.
    i tried two ways both not working

    [script type="text/javascript" src="http://www.allyscandle.com/index files/groupform.htm"[
    [/script]

    and the same way except groupform.js

    can anyone tell me what i am doing wrong?
    i even tried useing include and had no luck.
    i am totally lost and all the research i have been doing (even went to barnes and nobles and bought $200.00 in books) does not help me with anyhting but the calling of the file, nothing about writing the file. any good websites that give basic writing instructions?

    Instead of: < >

    If you are trying to include some html other then use some serverside script(if any) to include it.

    Like an <!--#include
    if so what do i have to put in the external file for that to work
