Need some help on installing Multiple Membership sites with affiliate

Hi!

I've been trying to install the Multiple memberships with affiliate script. It's installed, there are just a 1 or 2 errors I don't know what to do about.

Can somebody help me out?

(I can't get the verifying & referral url right)
