Captch not verifying within form handler.

I don't really know what the problem is, but my captcha is not verifying.

I added the necessary code to my form handler as instructed, but it doesn't require code be entered at all let alone correct code.

rather than post the code here now... is there anyone willing to help me diagnose the issue and lead me in the right direction for enabling the verification to occur.
