VB or C++ Simple MySQL Insert App

hi all :]

need a simple (doesnt gotta be pretty ;)) app that can sit on my desktop in VB or C++ that has several input fields and a button.. i fill the fields manually and click submit and it puts it into a remote mysql database.

I would need the code aswell as the application.

Please quote me price and time for this work, be about 5 or so fields.
