Need help on E-mail Form

Hi I'm new here and I just learned how to make a form but I'm having problem on php code, in my form I wanted ppl to enter their name, address, e-mail address, and questions; then send it to me by e-mail. I checked my hosting company and I know they support php code, I just don't know the php code, if anyone know php please help me out thanks :)
