Double combo MySql Populate

I am using the double combo script (http://www.javascriptkit.com/script/cut183.shtml).
I am a newbie in MySQL and PHP. Could anyone help me with the file to use in MyPHPadmin to populate my categories and subcategories?

you've got to learn the basics if you're going to get anywhere at all, surely phpadmin does this for via it's menus?

do some tutorials, you'll have to at some point and the earlier you do, the earlier you'll get the benefits
