need a database script for forum members to add bands to

I'm in search of a database script using php/mysql that will enable memebers at http://www.cassetteculture.net to add band names, urls and information to what I want to be the definitive list of independent music from the 1970's onwards.
Can anyone suggest a script?

bit of a vague question

simple to make an html form that collects the info and writes it to a db

presumably you then want that info displayed in some way?

are you goig to write this yourself? no biggie, fairly simple php/MYSQL

or are you looking for somethig off the shelf

are you going to integrate it into that phpbb forum?

I am not able to write it, I'd rather have something "off the shelf". If it could be integrated into my existing forum, that would be wonderful, but not absolutely necessary.

I've asked the question in the phpbb forum, but have not yet got a response.
