Script to make online certificates.

Hi, IM looking for a script that I can use to make award certificates on a web site I’m building. What I'm looking for exactly is when a user comes on to the site they can pick from a list of graphics using radio buttons i.e. vertical and horizontal borders and enter some text, then when they click submit their selection appears on another screen in a certificate image. Would appreciate any ideas or where I might find such a script.

have you written the HTML page to hold all the template info for the certificate. that's your first job! get that done, and the JS will be a piece of cake.
