needing help with instant quote generators

i'm fairly new to business site design and i'm working on designing a website for a DJ. he wants an ''instant quote generator'' like found here http://www.jplentertainment.djintel...asp?ID=weddings

i'm not AT ALL sure how to do this. i see the source code is set up like any other form layout but how would i do the quote from selection?
