I code from scratch with syn (http://syn.sf.net). I can't stand the bloat code the WYSIWYG editors produce  .

I love Homesite. I can't believe that I used Frontpage when I started out doing web dev.... :o Since I started coding XHTML/CSS and using CSS for layout, it has become so much easier than trying to code all the elements of a table in HTML..

one vote for "from scratch"!
I've used notepad from the beginning
From scratch. Every horrible design I've come up with so far was spawned from a sliced-up image and a text editor :D.
I've found notepad wholly inadequate. I can't really find a text editor somewhere between notepad and ultraedit (but closer to notepad) I really like. I find myself constantly switching between them as a result because using an IDE is often overkill but notepad (for example) simply doesn't cut it.

Hand coding, definitely. I think you'll get a pretty strong majority on that. But still most hand coding tools make things easier.
Textpad for Windows is my favorite. It has great Search & Replace functions that allow use of regular expressions. The customizable Clip Library and Macros make reusable code and repeated tasks pretty painless too.

I hand code... although since I've started using Fireworks I'll use that to make a mock up of the page in HTML after I'm done with the layout. This way I can get a mock online for the client to look through, push buttons, get a feel for things, etc. After that it's BBEdit all the way...

With hand coding, you get to make your site EXACTLY the way you want it. With every web design package I've used in design view, even the likes of Golive and dreamweaver, I have had things go wrong. However, its still possible to get loads of errors in your code if you hand code, but there is a chance you can get everything perfect when you hand code.
