well, within html itself, you're restricted to using the mailto: attribute which it in itself is not very convenient.

Other than that, this is a more CGI/Php related question, using a form. I'm sure if you searched for some in the threads you might find some results. If not, let me know, i'll try to look for some links that give you the code straight up.
