Form / Form Input Help, Inserting images into a form

I am trying to create a form that would allow someone to upload an image into it and it would be emailed to me with the image as an attachment.

Also, I would like to have the option of having a drawing script that someone could sketch a rough drawing and it would be included in the form also.

I am going to add this script to a website with a catalog. They will select a model of the product and upload or sketch the design they want on it, then it will be emailed to me with the order form so i can send them a price quote.

This site is exactly what I am looking for: custombuttons.com with the exception of the button product, I want to use it for another custom product.

Any help or ideas would be appreciated.
