Poll/Voting with addon

Gday fellas. (and chicks)

I am new to php and was wondering if someone could help me with a script similar to a regular voting/poll script (available freely everywhere). The difference with what I want is that i want to have the selectable answers to the question PLUS a text box for other opinions/answers. Then after that there will be a new answer to the question....
eg......

What is the greatest song of all time???

o SONG ONE - ARTIST ONE
o SONG TWO - ARTIST TWO
o OTHER |A TEXT BOX FOR USERS' INPUT (SONG THREE - ARTIST THREE)|

next time someone visits this poll,

the options will be

o SONG ONE - ARTIST ONE
o SONG TWO - ARTIST TWO
o SONG THREE - ARTIST THREE

the only thing that you need to do in the page that processes the form, is check if that textfield containes a value. If so, then insert it in the table where you pul your alternatives from.

if you need more help, then we need to know how your alternatives table looks like.
