how to create a .php file and how to connect the php file to the server?

A php file is essentially a plain text file and can be created using any text editor.
It should be placed on the server similarly to ordinary HTML files; setups should control that the php engine parses it before sending it to the requester.
But this all really should be in the PHP forums, should it not?
