I tried the above and it works great!

However, after placing the tag in all pages, I noticed that some pages worked while others did not.

Is there anything that would prevent the above tag to not work.

The whole favicon business is very sketchy in IE. Sometimes it works; sometimes it doesn't. Your tag is fine.
