Hi, i have a little problem. I have a form and want to send it per mail. But I don't know how to insert radio buttons or checkboxes. I only know how to insert textboxes. Can anyone help me??

Here is my script:

echo "Your survey was sent successfully to Mr.xx!\n\n";
echo $_REQUEST['Function']."<br>";
mail("my_mail@mail.com", "Survey form $Name", "$_REQUEST['Function']");

Do you mean in the initial php page or in the email itself? The former is easy, the latter I don't know about.

I know how to do it on the PHP page, but I want to send the Information in an email. Thats what I dont know!

there is this error all the time:

Parse error: parse error, expecting `T_STRING' or `T_VARIABLE' or `T_NUM_STRING' in /data/members/free/tripod/de/u/s/a/usaberliner/htdocs/form2.php on line 4

Try something like this by creating a $msg var and adding info to it using ".="

echo "Your survey was sent successfully to Mr.xx!\n\n";
echo $_REQUEST['Function']."<br>";
$msg = "";
$msg.= $_REQUEST['Function']."\n\n";
$msg.= "Add what ever you want here";
mail("my_mail@mail.com", "Survey form $Name", $msg);
