Help With Form/E-Mail Submit Button Error

As you can see, my "Tell A Friend" button is not working well... It should check for a real e-mail address and then open an e-mail. It worked, until I tried to add an image, rather than a button...

I entered my email....Says page not found...Just what do you need help with?

It used to work with a BUTTON, but I tried to replace that with a GRAPHIC BUTTON...
