explorer-style menu tree that drills down to a textarea

I'm seeking a Windows explorer-style menu tree that, once you've drilled down to the bottom of each branch, there is a textarea that the user can enter a request that will then be sent to an email recipient.

This is a web request form and my idea is to create a menu tree that mirrors our site navigation. If someone has an idea/request to update or change the content on any given page, they could drill down to that particular page and submit it in this fashion.

Any help would be greatly appreciated. I've searched a bit to no avail.
