If you are after a book I recommend:
PHP and MySQL Web Development
By Luke Welling & Laura Thompson
It starts with teaching you the basics of PHP then moves onto using MySQL with it. It also takes you step by step through basic examples of build a news script, mailing list duscussion board etc.
All the books code is on the CD along with Apache, PHP4 & MySql source & binaries!
Go to the books section of my site. You will find a link there to further details at Amazon.com
